Chronic Rhinosinusitis with Nasal Polyps

Rhinosinusitis is a disease characterized by inflammation of one or more of the paranasal sinuses. Chronic rhinosinusitis is divided into 2 clinical subtypes: (1) CRSwNP; (2) chronic rhinosinusitis without nasal polyps (CRSsNP). Nasal polyps are inflammatory outgrowths of the paranasal sinus mucosa along the middle and superior meatus that occur primarily in adults. In CRSwNP, NPs are benign and typically develop bilaterally in the sino-nasal cavity.



Treatment of NP includes medical and surgical therapy aimed at either complete elimination of polyps or sufficient reduction in polyp size to alleviate nasal obstruction and associated symptoms. Both topical corticosteroids and nasal saline irrigations are recommended as initial medical therapies in CRSwNP. Intranasal and oral corticosteroids can reduce NP size, lessen sino-nasal symptoms, and improve quality of life. However, the adverse effects of long-term systemic corticosteroid use outweigh their advantages and should only be considered for short- term administration.
